You can’t make bread without dough. In the same way, without B12, good health is impossible. Vitamin B12 (cobalamin) is one of those essential nutrients without which the body simply cannot function. As a water-soluble vitamin, it plays a key role in the formation of red blood cells, DNA synthesis and the maintenance of the nervous system — functions so fundamental that a severe and prolonged deficiency can have serious, even life-threatening, consequences. A deficiency, even a mild one, should therefore be taken seriously.
Why B12 is important
The body needs vitamin B12 to produce healthy red blood cells, synthesise DNA and ensure that nerve cells function properly. It also plays a role in energy metabolism by helping to convert food into usable energy.

Think of the standard B12 blood test as simply counting all the B12 circulating in the blood — but a large portion of this B12 is actually "stored" on a transport protein that doesn't deliver it to cells. Only a smaller fraction, carried by a different protein, is actually delivered to tissues and used by the body. So the number on your blood test may look perfectly normal, while the amount actually reaching your cells is insufficient — which explains why some people have real symptoms despite a "normal" B12 level on paper.
Or, to use another image: think of your B12 as money. The standard blood test counts all the money you have "on paper." But a good portion of that money is actually stuck in a kind of frozen account — it's there, but you can't use it. Only the part in your "checking account" can actually be spent — that is, in the case of B12, actually used by your cells.
This is exactly what happens in the body. A large part of the B12 circulating in the blood is bound to a protein that doesn't deliver it to cells. Only the other, smaller part is truly available and usable.
This is why you can have a perfectly "normal" blood test result while still genuinely lacking B12 where it matters: inside the cells. The standard test tells you how much you have on paper, not how much you can actually use.

How to recognise a high-quality B12 supplement

The precise quantity of each form: reliable labelling states the quantity in µg or mg of methylcobalamin and adenosylcobalamin separately, rather than simply listing their presence in the ingredients list.
Stability testing: adenosylcobalamin in particular is sensitive to light and heat. An amber glass bottle and documented stability tests are good indicators that the stated concentration remains reliable and stable.
Consistency between price and the cost of raw materials: adenosylcobalamin is a significantly more expensive raw material than cyanocobalamin. A product sold at a price well below the market average, whilst claiming to contain the same combination of methylcobalamin and adenosylcobalamin at the same dosage, warrants verification of the manufacturer’s sourcing and quality documentation.
